Fix for potentially undefined behavior in the Windows build, make the overloaded delete and delete[] operators check if the pointer is NULL before trying to use HeapFree(), thanks to Adam for pointing this out.

Fix the CMake generation to work with older versions of CMake 2.4.x, cleaning up some of the macros and also correcting the code to detect what version of CMake is in use.

Fixed issue with the MSVC++ build of Anope crashing on a DLL throwing an exception, thanks to w00t for pointing out the overloaded new/delete operators from InspIRCd.
