Don't dynamically allocate commands in modules anymore, instead made them members of modules. This means the commands are automatically destructed when the module is unloaded. Cleans up some old ugly code.

Used std::list for ignore's IgnoreData instead of using the old C-style double-linked list, also removed the addition of an ignore when a command "takes too long".

Epic commit to replace most of the strings in Anope with a single Anope::string class, plus some other little fixes here and there. If you follow 1.9.x development and are testing things, THIS is one of those things that NEEDS testing.
