Delay running the type creation event until after initialisation.

Doing this during the ctor can run into circumstances where the
vtable has not been fully initialised and the call gets routed to
the pure virtual implementation causing a crash.

Return references instead of pointers from the config system.

We used to return NULL from these methods but now we return an empty
block so this can never actually be null now.

Replace convertTo/stringify with non-throwing alternatives.
...
Having these throw is terrible for ergonomics and there are loads
of places where the exception was either silently ignored or not
handled at all. Having a function which returns an optional and
another that returns a default works a lot better imo.

Fix module event prioritization which was broken with the newer

auto-attach event stuff.
Also remove logically dead code from os_defcon, and make os_defcon akill
similar masks to os_session when enforcing session akills.
Fixes #1618 which relies on os_session to be prioritized before
os_defcon.
