Make the SASL PLAIN implementation more robust.

- Reject auth attempts that try to authenticate using an alternate
authorization identity.
- Reject auth attempts that contain extraneous null bytes in the
string as required by the SASL PLAIN RFC.
- General code quality cleanup.
